In 2015, Dr. Saiya Yokoyama published Keiseigeka wa Kando Geka (形成外科は感動外科), a book written to introduce the general public to the true scope and meaning of plastic and reconstructive surgery.

Drawing on patients and experiences from his career as a plastic surgeon, Dr. Yokoyama explores a remarkably broad range of medicine—from reconstruction following serious accidents and burns to cancer reconstruction, pediatric care, ptosis, aesthetic surgery, and complications caused by foreign materials.

Yet the book is ultimately about more than surgical procedures.

Its Japanese title, Keiseigeka wa Kando Geka, expresses Dr. Yokoyama’s belief that plastic surgery can be “surgery that moves the heart.” Restoring a part of the body damaged by injury or disease, improving function, or resolving a concern that has burdened someone for years can bring changes that extend far beyond physical appearance. Such moments can deeply affect not only patients and their families, but also the physicians who care for them.

For the English title, we express this idea as Plastic Surgery Changes Lives.

【専門医解説】鼻の手術のあと、瘢痕の増殖と改善について

最近はAIで得た回答をそのまま「正しい」と信じて気軽に投稿された情報が散見されますが、臨床経験がほとんどないか経験の浅い投稿者の投稿内容が検証されることなく正当性を纏って広がっているのは日常の光景となりました。それらの投稿が一般的に形成外科で共有されている知識や現場の肌感覚と乖離していることは珍しいことではありません。

こちらの記事は、30年以上にわたる臨床経験に基づいていますが、当院の特殊性「鼻の修正手術が8割、私の執刀、当院のアフターケアを受けられた患者様のデータ」という点をふまえてお読みください。

鼻の美容手術後の瘢痕増殖期について

一般的な創傷治癒学では、線維芽細胞の増殖やコラーゲン産生は術後数日から始まり、増殖期はおおよそ術後1~8週間と説明されます。

しかし、鼻形成術、特に鼻中隔延長術や修正手術では、患者様が実際に経験する「瘢痕の増殖」はこれより長期間に及びます。

8割が鼻の修正手術である私の臨床経験では、術後経過は次のように推移することが多いです。

術後5日~3週間:浮腫期

この時期は、瘢痕そのものよりも術後の浮腫の影響が主体です。

鼻全体が腫れ、硬さもありますが、その多くは浮腫によるものであり、本格的な瘢痕増殖とは区別して考えます。


術後3週間~4か月:臨床的な瘢痕増殖期

術後3週間頃から、浮腫が徐々に軽減する一方で、瘢痕組織の増殖が目立つようになります。

この時期には

  • 鼻尖が硬くなる
  • 可動性が低下する
  • つっぱり感が増す
  • 鼻先が厚ぼったく見える

といった症状が現れやすくなります。

瘢痕は術後3~4か月頃に最も多くなることが多く、これが臨床的なピークであると感じています。


術後4か月以降:増殖停止期

術後4か月頃になると、新たな瘢痕の増殖はほぼ落ち着きます。

ただし、この時点で瘢痕が消失するわけではなく、ここからは増殖ではなく成熟・再構築の段階へ移行します。


術後5か月以降:瘢痕軟化期

術後5か月頃から、瘢痕は徐々に柔らかくなり始めます。

一方で、この時期には個人差が大きく、

  • 5か月頃から改善する方
  • 6か月頃から改善が始まる方

など様々です。


術後1~1年半:瘢痕成熟期

瘢痕は少しずつ吸収・再構築され、

最終的に最も落ち着くのは術後1~1年半頃であることが多いと考えています。


最も重要なのは「4か月時点の瘢痕量」

臨床上、最も重要なのは術後4か月頃の瘢痕量です。

瘢痕は4か月頃をピークとして、その後徐々に減少していきますが、

ピーク時の瘢痕が多ければ、その後1年、あるいは1年半経過しても瘢痕が残存する可能性が高くなります。

逆に、ピーク時の瘢痕形成を少なく抑えられれば、その後の軟化や成熟も順調に進み、より自然な状態に近づきやすくなります。

そのため私は、術後4か月時点での瘢痕量を可能な限り少なくすることが、長期成績を左右する重要なポイントだと考えています。

これは修正手術だけでなく、初回手術においても同様です。組織への侵襲を最小限に抑え、血流を温存し、不必要な剥離や過度な緊張を避けることが、結果として1年後、さらには1年半後の鼻の柔らかさや自然な仕上がりにつながると考えています。


最後に

瘢痕のピークを下げる工夫は術後ケアだけではありません。

  • 組織への侵襲を最小限にする
  • 血流を温存する
  • 不必要な剥離や過度な緊張を避ける
  • 医師の指示のもと、テープ固定や圧迫固定を行う。服薬。

手術を丁寧に行うことは当然ですが、術後アフターケアをしっかりと行ってはじめて、鼻の手術は完結します。

ケナコルト注射についてQ&A

瘢痕が強い時期に不安でケナコルト注射を希望される方や、他院手術後の瘢痕増殖に対するケナコルト注射を希望される方がよくいらっしゃいます。よくあるご質問に対する回答です。*当院はケナコルトの代わりにリンデロン懸濁液(ステロイド)を使用していますがケナコルトに関するご相談が多いため、Q&Aを作成しました。

他院で術後にケナコルト注射を瘢痕に打ってもらったら凸凹になりました。修正手術は可能ですか

これは多いご相談です。ケナコルト注射は適切に実施しないと凸凹になります。安易に医師に依頼することも、医師が安易に使うことも、好ましくないです。修正の可否については診察が必要ですのでお越しください。

ケナコルト注射を打てば誰でも瘢痕がなくなりますか

ケナコルトの効果には個人差があります。安心のために打つ注射ではありません。打ち方によっては凸凹ができ取り返しがつきません。慎重にご検討ください。

真皮脂肪移植を受けましたが、ケナコルト注射を打ってもらえますか

投薬濃度、量によっては、注射した部位から近いところの真皮脂肪が吸収されます。真皮脂肪は次第に落ち着いてきますので焦らないようご注意ください。

他院で手術を受け2年が経過しました。ケナコルト注射を打って細くできますか

術後2年も経過していると、ケナコルト注射は効果がないどころか凹みます

これから修正手術を受けますが、鼻の瘢痕がすごいので修正前にケナコルト注射を打っておいた方がよいですか

修正手術の前にケナコルト注射で一時的にコントロールするということは通常行いません。

ケナコルト注射は、凹み、皮膚が薄くなる、稀に局所感染が起こるリスクがあります。手術結果の責任の所在が曖昧になるため、当院では対応していません。まずは執刀医の先生にご相談ください

他院でケナコルト注射を打ったところ皮膚が陥没してしまいました。修正する方法はありますか

陥没の状況にもよりますので、一度診察で拝見させてください。

ケナコルト注射には、皮膚が薄くなる、凸凹になる以外のリスク、合併症がありますか

他院でケナコルト注射を5回以上打ちましたが変わりません

ケナコルト注射はそう何度も打つものではありません。リスクを考えたら5回以上打つということはありませんし、5回以上打って合併症も出ず何も変わらないというのも考えられません。5回、10回打つというのはありえません。それはもはや気休めです。当院ではリンデロンは3回までです。

他院で何年もずっとケナコルト注射を打ってもらっています。このような治療を今後も執刀医にお願いしても大丈夫なのでしょうか

ケナコルト注射が有効なのは瘢痕が増減をする時期だけです。落ち着いた後にいくら打っても意味がありません。合併症が出るだけです。

ケナコルト注射を打ったところが陥没してしまったので、コンデンスリッチ脂肪を凹みに入れる提案をされました。入れてもよいのでしょうか。

ダメです

誰でもケナコルトを打てば瘢痕が減るというものではないのでしょうか

断った方が良い方もいます。見極めが大切です

私が手術を受けるクリニックは、鼻の手術のあとにケナコルト注射をするのがセットになっています。このような手術を受けても良いのでしょうか

そういうところはダメです

他院で複数回ケナコルトを注射した部位がありますが修正手術はしてもらえますか

ケナコルトを何度も注射したところは皮下組織が薄くなっています。術後に血行障害を発症し数日間は暗紫色になる可能性がありますが、手術はできます

当院でステロイド注射を希望される場合は、当院術後の患者様は3,300円/1回、他院術後の患者様は22,000円/1回の費用がかかります。繰り返しになりますが当院では経験上ケナコルトは使用いたしません。ステロイドも安易に打つ注射ではありませんが、希望される場合はご相談に応じます。
